Select a form (below) and it will be automatically loaded into Microsoft Word.
For Mozilla users: When the Download Manager comes up, select Launch File (top row, 4th selection from the left). In Microsoft Word select File, then Save As... and give the form a name. This form is now on your desktop.
For Netscape users: A Warning dialog box comes up asking if you want to open the form or save it to your disk. If you select Open it, then OK, the form is loaded into Microsoft Word and from there you can File then Save As... and name the file; you now have the form on your desktop. If you select Save it to desk, you must name the file first (in the Save As... window); in this case the file is saved to your desktop without bringing it up in Microsoft Word.
Internet Explorer Users: How to download the form:
Select a form (below).
When the form comes up in the browser, select File from the menu bar then select Save As... (make sure you rename the file); in this case the file is saved to your desktop without bringing it up in Microsoft Word.
Open Microsoft Word and select File then Open. When the Open window comes up, select the form you just downloaded.
Fill in the Form and Save it again before making a hard copy.
